REQUIREMENTS FOR PROVISIONAL AUTHORITY (PA)

Notarized Application Form

Form (5 copies) will be provided on-site. You may fill out upon application

Original and Photocopy of Proof of Citizenship (Birth Certificate and Valid ID)

Option 1: PSA Birth Certificate + any valid government-issued ID
Option 2: PSA Marriage Certificate + any valid government-issued ID
Option 3: Valid Philippine Passport ID

Original and Photocopy of Official Receipt (OR) and Certificate of Registration (CR)

Accepted Year Model: 2021-2024
If vehicle is less than 2 months from the date of purchase:
- Present OR, Sales Invoice and Delivery Receipt

If the applicant is not the owner of the vehicle:

Contract of Lease, if person; or
Certificate of Conformity, if encumbered or under financing; and
Valid ID of registered owner

If vehicle is under financing:

Original and photocopy of Certificate of Conformity from the bank
Original and photocopy of Delivery Receipt (DR) and Sales Invoice (SI)

If vehicle is named to another person:

owned by first degree relative (parent, child, spouse):

a) Original and photocopy of Notarized SPA with valid ID Marriage Certificate (if owned by spouse)
owned by peer/non-family member:

a) Original and photocopy of Notarized SPA with provision
to use the car as TNVS
b) Original and photocopy of Contract of Lease
2nd owner of the car:

a) Original and photocopy of Notarized Deed of Sale

License of the Driver-Applicant or Authorized Driver (If operator is the applicant)

Professional Driver's License

Proof of the Existence of Garage:

Google map location;
Photos of the Garage;
Copy of Transfer Certificate of Title/Tax Declaration in the name of the applicant;
Contract of Lease, if not owned

IF the applicant owns the garage:

Provide the Transfer Certicate of Title (TCT)/Tax Declaration in the name of the applicant (vehicle owner)
You can request Certicate of True Copy from the nearest Registry of Deeds or City Assessor of the local government where the property is located

IF the applicant leases the garage:

Submit a Notarized Contract of Lease/Authority to use with a copy of TCT/Tax Declation of Lessor

IF the applicant uses the garage for free:

Submit a photocopy of TCT, Tax Dec and Authorization from the owner

5R Photograph of unit:

With front page of a newspaper showing the date when thepictures were taken

DTI Business Name Registration

Make sure that there's ‘Transport’ in your Business Name (e.g. John’s Transport) and is registered at National level.

BIR Certificate of Registration or proof of filing (ITR 1701 or Certificate of Registration 2303)

Go to BIR to apply for a Certificate of Registration (Form 2303). Make sure to register to BIR within the same month you get the DTI Registration Certificate to avoid penalty.

Bank Deposit showing the amount of at least Php20,000.00

Can be requested from your bank minimum requirement amount is Php 20,000/unit.

Refund Policy for PA / CPC Fees

Refund Policy:
To get a refund, you must request it within 7 days of applying and before your application folder is submitted to LTFRB.A Php1,500 legal fee will be deducted from your refund.Lead time for claiming the refund would take 3-5 business days.
Important Note:
‍Once your application folder is submitted to LTFRB, the FULL fee becomes non-refundable.
